Funding Request Reports

We produce the following reports on a Weekly/Bi-Monthly/Monthly basis to assist our groups

in managing their Claim Account Balances.

 

Account Activity Log Summary

A summary report of deposits, paid claims, adjustments and account balance in date sequence.

 

Check Register

A detailed report of paid claims and adjustments since the last requested report in check number sequence.

 

Unpaid Claims Report

A detailed report of unpaid claims as of the report date in employee and claim number sequence.

 

 

Monthly Reports

Listed below are some of the most commonly requested reports that we produce each month

to assist our groups in managing their accounts.

 

Fund Account Statement

An account summary report by date including Opening Balance, Credits, Debits, Adjustments and Ending Balance.

 

Group Cash Receipts Report

A detailed report of all deposits and reversals to the account for the month including daily sub-totals.

 

Monthly Check Register

A monthly report of paid claims and adjustments in check number sequence.

 

Employee/Dependent Claims Analysis

A detailed report of claims information including amounts for total charge,

paid by plan, paid by employee, and discounts taken.

 

Benefit Utilization Review

A summary report of all Benefit Coverage Codes by Employer Group including

current month and year-to-date subtotals for each employee/dependent.

 

Claims Over Specific Analysis Report

A detailed year-to-date report of participant claims that, when totaled, are at 50%

or higher level of a specific reinsurance amount.  The report shows the Insured

and Claimant information along with the Amount Paid and other detail and

summary information for the selected Incurred Dates and Paid Dates.
